Admission Requirements and Enrollment Procedures

Contract must be read and signed by both parties
Parents are responsible for knowing material in this manual
Admission agreement form must be filled out
Child’s health history form must be submitted
Utah school immunization record form must be submitted

Trial Period
The trial period will be two weeks. This period is used to observe the child’s adjustment to care and to talk about concerns. We will talk to you daily about your child’s day. Please tell us if you have any concerns. After the two-week trial period, we will determine if the childcare services are satisfactory to everyone. If any problems cannot be resolved, the care is to be terminated.
